I will be at ... ..., Arizona
(street address city state)
(If living in the country, describe the exact location as if

I will remain quiet, in a prayerful, seeking attitude, hoping
that you can help me.
If possible, please ask the following questions at the end of
my Reading:
How much tubercular activity is there in my lungs at present?
Can it be cured, and if so, how?
Is there any other condition which is holding me back from
complete recovery?
What should I do from now on?
Would it be safe for me to start taking exercise, and can I
hope to be able to live in San Diego?

On opposite side please describe your case history. Over
I have had pulmonary tuberculosis for many years. For the
last six an a half years I have been in bed, with extensive
trouble in both lungs. Treatment has consisted of bilateral
pneumothorax, discontinued after about three years on account
of adhesions; temporary paralysis of phrenic nerve twice; and
finally, seven rib thoracoplasty three years ago and a
parafine pack (on other side from thoracoplasty) last summer.
Condition is now diagnosed as probably negative. The last
two laboratory reports were negative. The doctor says he
thinks I have nothing more to worry about, but he has thought
that before when events proved otherwise. I am still in bed
all of the time. Several times in the last six years I have
seemed well enough to get up and to try to do a few things.
Each time there has been a relapse.
